Wall Street Mailboxes has you covered. Come in person and we will fill out the PS Form 1583 for you. We can also do it virtually online. Download and fill out PS Form 1583 (required by the USPS). You will need two necessary forms of IDs.

The Checklist of what you will need.

Photo ID current (Not expired)
1. Most people use A state ID card, such as a driver’s license or Passport
Proof of Address Document Current
1. You can use your state ID card, such as a driver’s license. But can not be used as your Photo ID then.
If you want to add anyone to the mailbox or let anyone pickup your mail. They must sign a PS Form 1583 with you. They would also need Photo ID current (Not expired) and Proof of Address Document Current.

What forms of ID can be used for USPS Form 1583?

You can have your form notarized and mail it to us or schedule a Virtual signing session, you’ll also need to have two forms of identification ready.

The first form has to be one of the following:

A state ID card, such as a driver’s license
Passport (from the United States or another country) or passport card
U.S. permanent record card (aka green card)
Certificate of Naturalization
Armed Forces card
U.S. university ID card
Uniformed services ID card
Tribal identification card

The second form of identification (Proof of Address) can be an official document with your current mailing address. See below for what Proof of Address

Vehicle insurance policy
Home insurance policy
Lease
Mortgage
Deed of Trust
