How to Add a Name or Business

To ensure accurate mail sorting and compliance with USPS regulations, all recipients must be registered. Please have your mailbox number ready before starting.

Note: Submitting this form does not automatically update your mailbox. We will use this information to prepare your official PS 1583 form.

Step 2: Verification & Signing Once we process your data, you will need to sign the generated PS 1583 form.

• For Individuals: The person being added must appear in person to sign. You must bring the two forms of ID used to fill out the form.
• For Businesses: The authorized representative listed on the form must come in to sign.
• For Minors (Under 18): No ID is required for the minor. The parent/guardian must list them on the PS 1583 and sign a new form.
